Becoming a Patient at Trinity Chiropractic

 

WHAT IS YOUR NEW PATIENT PROCEDURE?

To make a new patient appointment at Trinity Chiropractic, fill out the contact form on the "contact" page of our website. You will be asked to fill out our online paperwork, as well as provide your insurance information so we may verify your coverage before your first visit. You must complete your new patient paperwork before your first appointment.

We are happy to serve patients without chiropractic insurance coverage. Call the office to discuss our affordable cash rates and packages.

WHAT SHOULD I EXPECT AT MY NEW PATIENT VISIT?

Your first appointment will consist of 4 steps: health history, examination, report of findings, and first adjustment. This appointment usually takes 45-60 minutes.

After you are an established patient at our office, appointments usually take around 10 minutes. We appreciate you choosing to include us into your busy schedule.

WHAT TYPES OF HEALTH INSURANCE DO YOU ACCEPT?

Dr. Kennedy is in network with Blue Cross Blue Shield. We will attempt to verify your insurance coverage before your first visit, but the best way to ensure your visits are covered by your plan is to call your insurance company. 

PLEASE NOTE: We are not in network with Blue Care Network (BCN). If your insurance card says HMO, be sure to call your insurance company to verify that Dr. Kennedy Cullen is in network. 

DO I NEED A DOCTOR’S REFERRAL?

No, you do not need a referral to make an appointment at Trinity Chiropractic. If you have insurance that we do not accept, you may still be able to receive partial coverage for your adjustments. Call your insurance company for more information.
